The Mammoth Lakes Fire Protection District prefers to utilize a paperless plan review and permitting process. All applications, including plans and technical documents, may be submitted in digital format via any of the following methods: (1) PDF emailed to firstname.lastname@example.org, (2) Dropbox or Google Drive link emailed to email@example.com, or (3) USB flash drive or CD delivered to the Prevention Division during regular business hours at Station 1, 3150 Main Street, Mammoth Lakes.
Please make sure your application packet is complete prior to submitting.
The installation of, work, or repair on any fire sprinkler alarm system shall be conducted by a C-10 certificated technician.
Project name, address, Assessor Parcel Number (APN), project owner's name, address and phone number, and name, title, address and phone number of design professional.
List of current applicable codes - e.g. "the plans declare the design uses 2019 72 National Fire Alarm and Signaling Code."
Occupancy group(s) and type of construction.
Gross square footage for each separate occupancy classification.
Detailed description of scope of work.
Plans must be signed and dated by the appropriate design professional.
Include name, address, phone number and license number of C-10 contractor.
Index of drawings with numbered pages.

Type of fire alarm system being installed (cellular, wireless, land-based)
Central station service
Labeled rooms with room dimensions.
The supervising station facility and UL number.
Type of alarm circuits indicated (Class A or B).
Device locations and mounting heights.
Location of fire alarm control panel.
Floor plan, with details to include:
Sectional views of structure, roof and ceiling, and rooms containing beam or solid joists.
The locations of partitions, non-rated walls, and rated walls. If not full height, indicate the heights of the wall and the ceiling.
The location of all equipment, devices, and appliances (including fire sprinkler control and test valves, fire smoke dampers, air handler units, magnetic door holders, etc.) and end-of-line devices.
The candela rating of each strobe.
Use of each room or space.
Riser diagram, with details to include:
Single-line wiring diagram (riser diagram) that shows the interconnection of each device and the system equipment.
Candela rating of each strobe.
Number of conductors in each wiring segment and the type and size of wire or conductor to be used.
The class and style for initiating, signaling line and notification device circuits, as well as circuit number or identification.
Calculations, with details to include:
Secondary power calculation which verifies that standby batteries, or other approved secondary power source, have 60 hours of battery backup or 24 hours with UL certification.
Voltage drop calculation which verifies that the voltage drop in the alarm notification circuits does not exceed 20 percent. Provide voltage drop calculations for each circuit.
Identification of fire alarm zones and description of annunciation zone assignments. For addressable devices, provide device addresses.
A key plan of the building and/or complex indicating the street location and the area of work within the building.
Sequence of Operation - Provide a description of the results of device activation, including details relating to annunciation, evacuation warning, remote signaling and activation of fire safety control functions. Also provide a programming description and label for each initiation, monitoring and control device.
Manufacturer's specification sheets and cut sheets for all devices, equipment, and materials to be used, including the transponder to the supervising station. Highlight on the cut sheet which device or equipment is being used, the listing information and the application per listing.
California State Fire Marshal (CSFM) listing services.
Provide the model, number, manufacturer's name, description, quantity, CSFM listing number, and symbols to be used (legend for each device, equipment, and conductors proposed to be installed).
